Process: 

Our practitioners are currently In Network with many health insurance companies!  To find out if we are in network with yours or to find out what your policy might cover: 

1. Text us at 508.362.3358 or email hello@ahon6a.com your name, email and phone number and we can send you a quick form to fill out.

2. Once this form is complete please let us know via call/ text so we can send your information over to our billing team. 

3. In 4-5 business days we will text you  with our findings. 

In a hurry? No problem, feel free to book a treatment, pay out of pocket and we will be happy to reimburse you once we hear back about your individual policy. 

 

Practitioners: 

Please keep in mind that not all our practitioners are in network with all health insurance companies, be sure to ask which practitioner accepts your insurance so you can always book appropriately.

I authorize my insurance company to pay my benefits directly to this office and I understand that I will be fully responsible for any outstanding balance on my account. THIS IS A DIRECT ASSIGNMENT OF MY RIGHTS AND BENEFITS UNDER THIS POLICY. I have been given the opportunity to pay my estimated deductible and coinsurance at the time of service and I agree to be personally responsible for the total charges incurred by me.
